Nurse Practitioner – Diabetes

The Nurse Practitioner (NP), Diabetes will function autonomously and collaboratively in an advanced and extended clinical role to provide safe, high quality advanced nursing care. This NP role will primarily perform nursing assessment, management, treatment and clinical support to patients referred to the role. The NP will have a focus on caring for children and adolescents with Diabetes (up to 18 years of age). The NP will contribute to the advancement of quality health care systems within the Local Health District to reduce avoidable hospital presentations and promote early discharge from hospital.
The core aspects of the role will be the delivery of safe, quality practice including the development of specialist plans of care that meet the needs of patients with diabetes in collaboration with the medical team.
The NP role includes the assessment and management of patients using nursing knowledge and skills and may include but is not limited to initiation of diagnostic investigations, prescribing of medications and direct referral of patients to other health care professionals. The NP will practice within the NP’s approved scope of practice (ScOP), competence, legislative and policy frameworks and the strategic direction of the service and CCLHD.
