Forensicare is the statutory agency responsible for the provision of adult forensic mental health services across Victoria. Completed in 2017, the new medium-security men’s prison is located in Ravenhall in Melbourne’s west, providing world-class forensic mental health services in Victoria in a purpose-built state of the art facility developed to international best practice standards.
About the role:
The AMHS Remand Liaison Coordinator is responsible for facilitating the continuity of mental health care from Ravenhall Correctional Centre to community based Area Mental Health Services (AMHS) for Forensicare patients on remand. The position provides guidance and support in transition planning and provides expert advice to assist Forensicare clinical staff with the AMHS linkage aspects of discharge planning for remand prisoners.
The role requires a high standard of interagency engagement with GEO Prison and Clinical staff, Correct Care Australasia staff and Alliance Partners.
You are a nurse, social worker or occupational therapist with experience and competency in planning, implementing and evaluating interventions and discharge plans with people with serious mental illnesses. You have a thorough understanding and demonstrated commitment to mental health consumer recovery, including the modelling and leading of recovery focused initiatives. You have the ability to build effective and collaborative working relationships with key stakeholders and to as an independent clinician across a number of mental health teams.
Strong knowledge of, and linkages with Victorian Area Mental Health services is highly desirable.
Mandatory qualifications & registrations:
Approved qualification in nursing, social work or occupational therapy and current registration with AHPRA where relevant. Social Workers must be eligible for membership with the AASW.
