Overview
This microcredential is the first section in the Chronic Disease Management in the Community series. The purpose of this microcredential is to explore the basic concepts of chronic disease in community settings. Utilising the knowledge of chronic disease epidemiology, environmental factors’ impact, and the human experience of disease in assessment practice. It aims to integrate those basic concepts and principles in assessing people with chronic disease and complex care needs.
This microcredential is the first in a series of three that corresponds to the unit of study 339 Chronic Disease Management in the Community.
Entry requirements
Applicable to all registered nurses who work with people with chronic disease.
Scope of future study
This microcredential can be completed to receive a standalone credential or stacked together in a series of three microcredentials. Completing all three microcredentials in the series allows you to apply for credit towards the unit 339 Chronic Disease Management in the Community (6 credit points) to build towards a full Graduate Certificate in Community and Primary Health Nursing at ACN.

Learning outcomes
- appraise the evidence-based policies, models and guidelines that inform chronic disease management in Australia
- analyse the impact of the illness trajectory on people experiencing chronic disease
- appraise the chronic health issues experienced by First Nations Peoples and people from culturally and linguistically diverse background
- analyse the health literacy and communication skills needed by individuals to increase capacity in self-management of chronic disease

» About me
Sandra is an experienced registered nurse and holds a Master’s in Nursing Advance Practice from the University of Newcastle. Prior to joining the Australian College of Nursing, she worked as a clinical nurse specialist and nurse educator in busy acute settings, providing expert guidance and education to nursing staff. In additional to her clinical education experience, she has also been actively involved in the education of undergraduate students in the university setting. Sandra has a particular interest in responding to and managing clinical deterioration and supporting nursing staff in this important aspect of clinical care.
Sandra believes in fostering a culture of safety through collaborative patient care, continuous improvement, critical thinking and bridging the gap between theory and practice. Her passion is providing education that nurtures and guides other nurses to become well equipped expert practitioners.
