Overview
The Chronic Disease Assessment microcredential is the first section in the Chronic Disease Management in the Community series. The purpose of this microcredential is to explore the basic concepts of chronic disease in community settings. Utilising the knowledge of chronic disease epidemiology, environmental factors’ impact, and the human experience of disease in assessment practice. It aims to integrate those basic concepts and principles in assessing people with chronic disease and complex care needs.

Learning outcomes
- appraise the evidence-based policies, models and guidelines that inform chronic disease management in Australia
- analyse the impact of the illness trajectory on people experiencing chronic disease
- critically evaluate the needs of people with chronic disease to develop strategies that are person-centred and holistic
- appraise the chronic health issues experienced by First Nations Peoples and people from culturally and linguistically diverse background
- critically analyse the health literacy and communication skills needed by individuals to increase capacity in self-management of chronic disease

Linlin has worked in the primary health care settings since the beginning of her career. She has achieved multiple qualifications that would benefit her as a nurse in primary health care setting, such as Immunisation for Health Practitioners, Well Women’s screening, Cannulation for nurses and Wound closure and Suturing.
Linlin has a passion for community and primary health care nursing education. She believes community healthcare is the fundamental component of our medical system and primary healthcare is the client’s first encounter with our health sectors.
